Facility Use Form
Thank you for your interest in the use of Licking Valley facilities. Our buildings are an asset to the LV Community and a number of civic and community groups use our facilities for meetings and other events. We are happy to accommodate these groups whenever possible. There are a number of guidelines for requesting use of Licking Valley facilities, as well as building use fees in certain situations. These guidelines can be found by searching the Administrative Guidelines on the Board of Education Board Policies page of our web site for section 7510A - Use of District Premises.

Register For A New Account (only needed for first time use)
All facility requests are completed online. Be sure to complete all information fields and then submit your application. If your registration is successful you'll receive and email confirmation.
